(IKC) is looking for a Full-Time Nurse Practitioner (NP) to join us
in Des Moines, IA. No Call, No Weekends, No Nights! Full Time, self
managed schedule!As an IKC Nurse Practitioner (NP), you will
support some of our most complex patients, assisting them in
navigating a challenging healthcare system. You'll focus on
delivering holistic, integrated care to improve the quality of life
for our kidney patients while also integrating primary care
responsibilities to meet their broader healthcare needs. You'll
work autonomously and in collaboration with all members of the
healthcare team to coordinate and facilitate quality, consistent,
cost-effective care.Position Details
- Location: Based out of home office and travel within an
assigned geography, including Dialysis Clinics, Nephrology
Practices.
- Primary Care Responsibilities:
- Perform routine health maintenance exams, wellness checks, and
preventative care screenings for patients.
- Manage common primary care conditions such as hypertension,
diabetes, hyperlipidemia, congestive heart failure, asthma, and
chronic obstructive pulmonary disease (COPD) within the context of
the patient's overall health and kidney care.
- Offer patient education on lifestyle modifications, preventive
care strategies, and disease management for chronic
conditions.
- Prescribe and monitor medications for common primary care
conditions and adjust treatment plans as necessary.
- Collaborate with other healthcare providers for ongoing patient
care, including managing referrals to specialists when
required.
- Ensure timely follow-up for preventive care and chronic disease
management, including vaccinations, screenings, and routine lab
work.
- Complete Comprehensive Health Assessments
- Manage Complex Patients: Focus on reducing hospitalizations and
improving health outcomes.
- Monday-Friday Schedule: Ability to accommodate patient and
nephrology partner's availability. Must be flexible.

- Passion for delivering care to patients with complex, chronic
illnesses and integrating primary care into the treatment
plan.
- Minimum of two (2) years' experience as a Nurse Practitioner
(NP).
- Current unrestricted NP license to practice in the state.
- Current Drug Enforcement Administration (DEA) license required
or eligible to obtain within 90 days of hire.
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life
Support (BLS) certification.
- Intermediate computer skills and proficiency in MS Word, Excel
& Outlook required.
- Home office, high-speed internet, and reliable transportation
required.Preferred Qualifications
- Preferred experience working in Value-Based Care, including
documenting and coding from a Medicare Advantage (MA) risk
perspective and conducting Health Risk Assessments on
high-risk/chronically ill patients.
- 5+ years' experience as a Family Nurse Practitioner (FNP),
Primary Care Nurse Practitioner (NP), or experience in Internal
Medicine, Cardiology and Nephrology.
- Spanish-speaking preferred in some markets.
- Experience in managing chronic diseases such as CHF, diabetes,
hypertension, and other conditions typically addressed in primary
care.
- Experience in managing population health, including reducing
hospitalizations and improving chronic disease management
